Hugo Broos' side held on as Ghana fought back and eventually sealed the result in stoppage time.
Christian Bassagog combined with Aboubakar on the counter, to finish calmly past Brimah.
The Indomitable Lions will now tackle Egypt for the trophy, after the Pharoahs saw off Burkina Faso via penalty kicks.
